What is NDIS community nursing?

NDIS community nursing is nursing care delivered in your home or community when it is disability-related and included in your NDIS plan.

It can include wound care, continence and catheter support, medication support, health monitoring, and education for participants and families.

With consent, it may also include coordination with your GP, hospital, and allied health team.

Who this is for

  5. Service commences, with review check-ins as needs change.

Funding for NDIS Community Nursing

NDIS community nursing may be funded when the nursing support is disability-related and included in your plan. If you are unsure which budget category applies, we can review your current plan details during intake and advise what information your Support Coordinator or Plan Manager may need.

Common funding pathway

Nursing supports are often delivered through Capacity Building supports when they relate to disability-related health needs and are included in the plan.

What we may ask for

To confirm eligibility and start quickly, we may request your current NDIS plan, goals, and any relevant clinical information (for example discharge summary or current nursing care plan), with consent.

For Support Coordinators and hospitals

We can provide a clear scope of supports, recommended visit frequency, and intake notes to help referrals, rostering, and continuity of care.
